PENYAKIT FAHR’S DENGAN KEJANG PADA PASIEN KARSINOMA TIROID: SEBUAH LAPORAN KASUS

Abstract

ABSTRAK

Penyakit Fahr’s adalah penyakit dengan gangguan neuropsikiatri yang ditandai dengan kalsifikasi di ganglia basalis bilateral dari hasil pemeriksaan pencitraan otak dan biasanya berhubungan dengan gangguan metabolisme fosfor dan kalsium. Artikel ini menyajikan kasus penyakit Fahr’s pada seorang wanita usia 56 tahun dengan karsinoma tiroid dan kejang. Selanjutnya, CT scan kepala menunjukkan adanya kalsifikasi  yang simetris bilateral pada ganglia basalis, dan otak kecil. Manifestasi klinis penyakit Fahr’s bervariasi tergantung usia, letak bagian otak yang mengalami kalsifikasi, dalam hal ini gejala yang dominan adalah jenis kejang berulang berupa bangkitan general tonic clonic. Kejang yang terjadi dalam hal ini bisa jadi akibat perubahan hormon tiroid atau akibat kalsifikasi di otak. Belum ada pengobatan khusus pada penyakit ini,.Gejala kejang yang terjadi mendapatkan terapi dengan sodium valproate dan terapi untuk gangguan perubahan hormon tiroidnya.

ABSTRAK

Fahr’s disease is a disease with neuropsychiatric disorder, characterized by bilateral basal ganglia calcification and usually associated with a phosphorus and calcium metabolism disorder. This paper presents a case of Fahr’s disease in a 56-year-old woman with carcinoma thyroid and seizures. Furthermore, CT showed bilateral symmetric basal ganglia and cerebellum calcifications. Clinical manifestations of Fahr’s disease vary depending on age, location of the part of the brain that is calcified, in this case the dominant symptom is recurrent seizures type of the general tonic clonic. Seizures that occur in this case can be resulted of thyroid hormone changes or brain calcification. There is no specific treatment for this disease. The seizures that occur got therapy with sodium valproate and therapy for disorders of thyroid hormone changes.
